MK19 Blown in place

Had a little incident with a MK19 here in Afghan.
Was doing some training with some of my guys...3 round bursts, last round detonated...sending shrapnel every where....luckily sandbags were stacked right and shrapnel went in the bags, not us.
Well HE tip lodged in barrel, and was a hot barrel, shredding the inside.
Well blew the gun....after all said and done, we believe it was bad ammo...
ammo is swelling in the heat causing he tip to unseat itself from casing...
lesson learned, use fresh ammo straight from the ammo can.
