Never lie. Period.
The truth will come out and then you will be dealing with an issue of reliability and trustworthiness.
Disclose everything, provide documentation as requested. Be ready to go see numerous specialists ( on your dime ) and accept the decisions made at MEPS.
It is far better to be honest and accept the decision than to lie and have it come out later. Imagine what a future Team Sergeant would do to you when they find out you lied on medical forms just to get into the service?
Team Sergeant scares the crap out of me and I know there are at least 1000 miles between us at any given time. I don't ever want him or any other QP angry at me for any reason.
Tell the truth or you will face the wrath of a supervisor some time in the future.
