Looking for a Safety Mishap Report.

Circa 2006/2007 there was an incident in Afghanistan in which several ODA members were injured by fragmentation while conducting explosive breaching training utilizing a metallic linear shaped charge. I know there was a Summary of Mishaps published for the incident and I am trying to locate a copy of it for some research I am doing. Does anyone remember the incident in question and/or have a copy of the report? Thanks.

Try contacting the 1st SFC(A) G-33 they should be able to pull up any archives on OPREPs or message traffic (not sure of who’s there now).

Also you can contact the USASOC Safety office, they should have the reports for any safety/ training mishaps.

I was there during 2006 as a Team Sgt; do you have any other specifics? I can reach out to others that were there during that time.

I will give them a try but not sure how willing they will be to help out a retired E7.

I read the safety report when it came out but never kept a copy. I might also be a year or two off in either direction time wise. IIRC, it was a 3 SFG(A) team. They ended up eating a bunch of frag and I believe they were not wearing proper PPE which made it all the worse.
