The entire Joint Staff is a disgrace. I don't know what's worse: the "we never saw it coming" excuse, or the "we knew this was going to be a chit-show" explanation. Either way, they all need to resign or be fired.
When Milley was CDR FORSCOM, I had to sit in on an accident brief wherein a young troop was tragically killed. The information demands of this clown were insatiable (some of the demands understood), but it went four hours.
"I want to see the NCOER's of the ground guide that night"
"I want to know the ration scheduling"
I want to know the state of cleanliness of the weapons;
etc...etc...etc.
Funny how he relieves himself of those very same requirements when HE is in charge. Fat bastard.

Bottom line up front.
If they didn't know this would happen they were wrong.
If they knew it would happen they didn't speak out so they were wrong again.
Either way they were wrong and should be fired - but they make Joe look good so he'll keep them.

IMHO, in one word, careerism.
I am from the same era as Milley.
When I was Infantry in the early-mid 80s, I was told by my branch manager that one tour in SF would not be a problem, but more than that and I was ruining a perfectly good career.
When word came down in 1986 or so that SF was branching, and SF officers could opt to become 18As or return to their basic branch, people had to make decisions.
We had one officer in the battalion choose to return to the Infantry. All the rest went SF.
When I asked him why, he said that the competition for promotion would be much less, and he was going to be a general.
He went down the street to the 82nd and made it all the way to O-5 or O-6 before he was killed in a helicopter crash.

I know at least four that returned to their Infantry roots and made GO.
