How to determine medic levels?

I am interested in the SF and planning on joining the local guard unit to work toward this goal.My questions is how is the medical level chosen?

Or does the SF just put you in a slot that is needed?

I am planning on going for 91-w.With possible secondary MOS as 11-b.

You have quite a bit of research to do my friend. You can find many of your answers here...but you will have to put yourown effort into it.
