The increased incidence of birth defects from closely related pairings is about 2-3% in the first generation. The issue with many Pakistanis is they've been doing this for generation after generation.

I know a doctor in the UK who started an outreach program in Leeds in the '90s among the Pakistani immigrants. They were shocked to learn that their marriage practices could be contributing to the high rates of genetic defects in their community.

They were seeing some success but the UK govt quashed the program. It apparently didn't accommodate diversity in the Muslim community.
