As a Civic Action dispensary and during MEDCAPS in SEA 35 years ago, our average WHO funded patient load for various Dx and population control programs was between 100-150 per day - and around 20% of those we saw and Rx were TB related...with nearly 100% of them also having co-related vitamin deficiency Dxs. Reasonable precautions and good health on our part resulted in none of us ever having a positive IPPD. 
OTOH - AIDS was not an issue then...
