Were the kits recently cleaned?

Sounds like a stuck firing pin to me.

5.56 conversion or 9mm generic?

I agree with Reaper. We shoot a metric buttload of the stuff and I can't remember it ever happening. They clean them after every use just like the other guns.

If you only tried a few times. Keep repeating what he did and try to replicate it. Unless the weapon has since been cleaned or lubricated. The standard of removing someone from training because of an AD. Is the one standard that can't have two different outcomes. No matter who it is.
If he remains in training and is involved in another incident. No matter how small. Someone will have a lot of explaining to do. The next time someone has an AD. It could be next week or next year. They will bring up this AD and it's outcome, guaranteed. I have seen this first hand bite people in the Ass. The next guy might not be such a good guy.
I have seen great guys have AD's. No matter how difficult it was to make them turn their Gear in. We were always reminded later, that it was the right thing to do.
