Can you be a bit more specific in what you are looking for?
There are "apps" for almost everything now, I use medical apps on my iPhone daily in my medical practice.
What capabilities are you looking for regarding the "tele" part? Audio only? Audio and visual transmission in real time? Phone, WiFi or Sat services? Secure or unsecure?
These details will narrow down, or expand greatly, the platforms you have available.
