Problem Observation: I see someone that tried to stack and blew it?? Why no cut-a-way??

Depends on how low they were when the cut-away option was discussed. And if jumper "a" had a perfectly good chute and jumper "b" was perfectly tangled then why risk a cut-away.

Also we cannot see how tangled "b" is and his entanglement might also cover his reserve chute and seen by "a".

This seems like a monumental mis-reporting of an incident. If jumper one had a bad malfunction, it would have been almost impossible for jumper two to catch him. This looks like a wrap in a stack. No criticism of the handling of the wrap as both survived but they may have been indulging in CRW below safe altitude. Hope thy were wearing floatation gear.
